Start here: open the bag and read the guaranteed analysis and the ingredient list before you decide anything based on the word “grain-free” printed on the front. That single word tells you almost nothing about protein quality, calorie density, or whether the diet is right for your particular dog. It tells you only that wheat, corn, rice, oats, and barley have been swapped for something else — usually potatoes, peas, or lentils.

In my practice, I’ve watched this play out more times than I can count: an owner switches a healthy adult Labrador from a grain-inclusive kibble to a grain-free one because a neighbor swore it “fixed” their dog’s itching, and six months later the dog’s coat looks the same, the stool is looser, and nobody can say why. I had a client bring in a seven-year-old mixed-breed, about 62 pounds, whose energy had dropped over roughly four months — slower on stairs, reluctant to jump onto the tailgate of the truck. That matters because it means grain-free is not inherently “more natural” or “closer to ancestral” — pulses and potatoes are still processed starches, and in some formulas they show up in larger total quantity than the grains they replaced.

Why the DCM investigation changed how I read labels

The reason I ask about grain-free history in nearly every diet consult now traces back to an FDA investigation that began in 2018. Between January 2018 and April 2019, the agency logged 553 reports of dogs with dilated cardiomyopathy — a heart muscle disease — compared with a historical baseline of zero to three reports a year. More than 90% of the diets involved were labeled grain-free, and 93% of those diets listed peas or lentils among the top ingredients. Reports have since declined substantially, and the agency has said the picture is a complex one that likely involves multiple contributing factors rather than a single ingredient. That’s an honest, unresolved answer, and I think owners deserve to hear it stated that way rather than as a verdict in either direction.

What I take from it clinically isn’t “never feed grain-free.” It’s that a legume-heavy, grain-free diet fed long-term to a large or giant breed deserves a periodic conversation with your vet about cardiac screening, particularly if the dog is a breed with any predisposition to DCM. Weigh what a diet swap can realistically do for a dog’s heart and digestive health before treating it as a low-stakes preference call — it rarely is one.

Where digestion, not the heart, is the real deciding factor

For most healthy dogs, the grain-free-versus-grain-inclusive decision has less to do with cardiac risk and more to do with the gut. Grains like rice are often lower in fiber and easier on a sensitive stomach than some legume-forward grain-free formulas, which can carry more fermentable fiber and produce softer stool in dogs that don’t tolerate it well.

This is where structure/function nutrition earns its keep rather than marketing language. Fiber sources matter more than whether they came from a grain or a legume. Pumpkin fiber, for instance, is a gentle, natural source of both soluble and insoluble fiber, and it’s a reasonable add-in regardless of which base diet a dog is on. A balanced gut microbiome supports overall immune function, and getting there is more about fiber diversity and consistency than about avoiding a single ingredient category.

Dogs with diagnosed exocrine pancreatic insufficiency are a clearer case where diet composition genuinely matters clinically: EPI is treated by pancreatic enzyme replacement therapy, nutritional management with low-residue diets of moderate fat content, and cobalamin supplementation, according to a review published in JAVMA. That same review notes that researchers still don’t fully understand why some dogs with EPI develop persistent gut dysbiosis even after enzyme therapy begins — a reminder that “the right diet” is often diagnosis-specific, not a blanket grain-free-or-not choice.

What actually predicts whether a diet will work for your dog

Probiotic response is individual, too. A 2025 randomized trial in Frontiers in Veterinary Science found that diversity metrics in the gut microbiome didn’t distinguish dogs who responded to a probiotic intervention from those who didn’t — which tells me, as a nutritionist, that broad microbiome measures aren’t yet good predictors of which individual dog will do well on which formula. That’s exactly why I resist blanket rules about grain versus no-grain. Some multi-strain products combine probiotics, prebiotic fiber, a postbiotic yeast complex, and digestive enzymes into one formula, and for a dog with a touchy gut, that combination can matter more than the grain question ever will.

For a healthy adult dog with no history of heart disease or grain sensitivity, either category can be part of a complete and balanced diet, and the ingredient quality within the category matters more than the category itself. For large and giant breeds on long-term, legume-heavy grain-free diets, I ask owners to keep cardiac screening on the schedule and to have an honest conversation with their vet about whether the diet still makes sense. Individual dogs vary widely, and the food that keeps one dog’s stool solid and coat glossy may do neither for the next — that’s not a failure of either category, it’s just how nutrition works.
